Understanding posterior tibial tendon
The posterior tibial tendon supports the arch. Degeneration or tearing can cause pain and gradual flattening of the foot, sometimes progressing to stiffness and arthritis. This condition may also be described as progressive collapsing foot deformity because structures beyond the tendon can contribute as it advances.
Common signs and symptoms
- Pain behind the inner ankle
- Loss of arch height
- Weakness with a single-leg heel rise
- Increasing foot fatigue
What to expect during evaluation
Evaluation includes standing alignment, tendon strength, flexibility, gait, and weight-bearing imaging. MRI may be used when tendon tearing or surgical planning is a concern.
Treatment options
Treatment depends on the diagnosis, severity, activity goals, overall health, and prior care. Options may include:
- Supportive footwear
- Custom orthotics or bracing
- Physical therapy
- Temporary immobilization
- Tendon and alignment reconstruction when indicated
