Understanding lisfranc injuries

A Lisfranc injury affects the joints and stabilizing ligaments of the midfoot. These injuries may follow high-energy trauma or a subtle twist and can be missed without weight-bearing evaluation.

Common signs and symptoms

  • Midfoot pain and swelling
  • Bruising beneath the foot
  • Pain with push-off
  • Difficulty bearing weight

What to expect during evaluation

Weight-bearing X-rays compare joint alignment. CT or MRI may define fractures, subtle instability, or ligament damage.

Treatment options

Treatment depends on the diagnosis, severity, activity goals, overall health, and prior care. Options may include:

  • Strict protection for stable injuries
  • Casting or boot immobilization
  • Repeat weight-bearing imaging
  • Surgical fixation
  • Primary fusion for selected unstable patterns
When to seek prompt care: Seek prompt care for severe pain, deformity, an open wound, rapidly increasing swelling, spreading redness, loss of sensation, or a cool or discolored foot.
