Understanding achilles tendinitis
Achilles tendinitis may affect the middle of the tendon or its insertion at the heel. The broader term Achilles tendinopathy is also used because chronic symptoms often involve tendon degeneration rather than inflammation alone. Overuse, tightness, training changes, and foot mechanics can all contribute.
Common signs and symptoms
- Morning stiffness behind the ankle
- Tenderness or thickening of the tendon
- Pain with running or stairs
- Swelling near the heel
What to expect during evaluation
The examination assesses tendon continuity, calf strength, ankle motion, foot alignment, and the precise area of pain. X-rays, ultrasound, or MRI may be recommended for insertional disease or suspected tearing.
Treatment options
Treatment depends on the diagnosis, severity, activity goals, overall health, and prior care. Options may include:
- Activity modification
- Heel lifts or temporary immobilization
- Structured physical therapy
- Footwear or orthotic support
- Debridement or reconstruction when nonsurgical care fails
